Summary: Do you have Crohn’s disease or ulcerative colitis?

If you answer "yes," you may be able to help researchers at UK who are trying to locate close relatives of patients like you.

You are eligible to participate in our survey if you:

• Have Crohn’s disease or ulcerative colitis; are age 18 or older;
• are not currently in jail or prison;
• would like for us to contact by mail the relatives you name on your survey to see if they would like to answer health questions for our research study.
